Every
coach and Team Parent volunteer must have a risk management pass on hand
at practices and games. The following is the process to obtain a pass.
| 1) | DOWNLOAD FORM from BSC website |
| 2) | Complete form and attach picture |
| 3) | Mail to Bloomfield SC Risk Management Coordinator: |
| Pam Nakoski 3416 Taft Road Bloomfield, NY 14469 |

| 4) | Pam will then consolidate and attach a Bloomfield Soccer Club check and send package to NYSWYSA. |
| 5) | NYSWYSA will then return the passes to Pam for lamentation and she will distribute to coaches. |
